Should I stay in the car?

On a fast stretch of the A30 or the M25 slip, get behind the barrier if there is one. On a quiet GU25 lane, staying with the vehicle is usually fine. Tell us which it is so we know how to approach.

How long at night?

It depends where you are relative to the van, not the clock. Give the landmark. We will give a window, not a fantasy.

What if I only have a doughnut spare?

If you can fit it safely, do. If you cannot, wait. We would rather fit a proper tyre than watch a space-saver fail two miles later.
